Personal Information Collection Statement
1. Petworld Resort will only use your personal information for providing our services and will not sell, rent or trade your personal information to any third parties except as required by law or permitted under the Personal Data (Privacy) Ordinance (Cap. 486).
2. In the event that any such personal information is not provided, we may not be able to provide you any services.
3. Under the Personal Data (Privacy) Ordinance, you have the right to request access correct your personal information. You may submit a written request to us for the aforesaid if necessary.
I understand I will be responsible for placing the pet into the cage and taking the pet from the cage by myself.
Will your dog need social time during the stay? (Dog who’s not neutered or being aggressive toward other dogs is not allowed to join the social time.)
** Evaluation will be done on the check in to decide if your pet needs individual walk, surcharge will be applied for individual walk.
(Additional charge will be applied for grooming service; for dog who’s boarding at Petworld Resort for over 7days is requested to have at least one shower)
Would you like your pet to swim during the stay?
(Additional charge will be applied for swimming day care; it includes 45mins swimming session and basic grooming service. Charges from $ 400 - $ 1200/time)

I understand that my pet could suffer from stress due to a sudden change and of living environment and eating habit, my pets may also have symptoms on separation anxiety resulting in sickness or destructive behaviors whereas may result in injuries.
I understand my pet might suffer kennel cough or skin issue due to the change of the environment or the weather
I understand that my pet may occasionally have the following hidden health problems: Defects in organs like heart, liver, kidney, pancreas, spleen, etc,;
Latent tumor / cancer, tick fever, heartworm disease

** Please be reminded that all the vaccines should be valid during the boarding period
** Your pet should have the annual vaccine (Every year), kennel cough vaccine (Every year) and Rabies shot (Every 3 years) to meet with our boarding requirement.
